Scope of Care: Contract Hospitalists, as appropriate and within scope of practice/privileging, shall be responsible for providing general medicine care to hospitalized patients, including, but not limited to:Provide medical care as needed on all units, including at Castle Point (E-2 Acute Med/Telemetry/ Medical Observation and H-1 Extended Care Unit) and at Montrose (4AB/Acute Psychiatry Unit; 6AB , 6CD and 15AB /Extended Care units; and 28/52/15CD /Residential Care Units).Evaluation and management of Clinical issues and changes arising during the coverage period.Round on each patient on the E-2 Acute Med/Telemetry/ Medical Observation on the weekends and Holidays. For H-1 Extended Care Unit at Castle Point and at Montrose 4AB Acute Psychiatry Unit, Extended Care Units including 15AB, 6AB and 6CD documentation for changes in Clinical status is required.All inpatients (Medical and Psychiatric) will be evaluated at their bedside in a timely manner. Whenever there is a new complaint, a change in the Clinical condition of the patient, or the nurse calls the Hospitalist, the Hospitalist must physically assess the patient and document the findings electronically.Urgent Room or inpatient consultations will be handled as follows: Urgent Room at Montrose will be open from 8am-8pm and 8am-6pm on weekends and holidays. Urgent Room at Castle Point will be open from 8am-8pm and 8am-6pm on weekends and holidays.The Hospitalist will evaluate the patient and determine who should be admitted to E-2 Acute Med/Telemetry/ Medical Observation.If admission is indicated, the Hospitalist on-call will evaluate/admit the patient. Contract Hospitalist will be fully cognizant of procedures concerned with the determination of blood compatibility in blood transfusions and will be responsible for verification of compatibility. Contract Hospitalist shall be familiar with Medical Center Disaster Policy and is responsible for treatment of casualties until the arrival of additional VA medical staff and it is deemed that the contract Hospitalist no longer needed.Contract Hospitalist shall be familiar with Code Blue Policy. Contract Hospitalist shall screen calls from private and VA Hospitals regarding referral and/or transfer of patients to other Medical Centers.Duties for Community Living Center coverageRounds, admissions or discharges are NOT part of the coverage on the nursing home/Community Living Center.Contract Hospitalists will respond by phone to urgent calls that cannot wait until the attending of record returns the next business day.If acute admission is deemed necessary, arrangements will be made directly with the nursing team and MAA.Telephone consultation with the Geriatric Department Attending on-call is availableMedications: All contract Hospitalist shall follow all established medication policies and procedures. No sample medications shall be provided to patients.Discharge education: All contract Hospitalist shall provide discharge education and follow up instructions that are coordinated with the next care setting, if required.ADMINISTRATIVE: 10% of time not involved in direct patient careQuality Assurance/Quality Improvement (QA/QI) Documentation: The contract Hospitalist shall participate in continuous quality improvement activities and occasional as required by the VAMC Chief of Service, Chief of Staff, or designee.Meetings: When requested, the LEAD Hospitalist shall attend staff meetings as required. LEAD to communicate with COR and report any conflicts that may interfere with compliance of this requirement. LEAD is required to attend all Quality Assurance meetings.QA/QI Documentation: The contract Hospitalist shall complete the appropriate QM documentation pertaining to all procedures, complications, and outcome of examinations.Specialized CPRS Documentation Procedures: Patient documentation is completed using the Computerized Patient Record System (CPRS). All contract Hospitalist shall participate in a training session prior to providing any patient care services at VA. Contract Hospitalist shall document care provided and select appropriate Evaluation and Management (E&M) procedure codes in CPRS. Documentation must be sufficient to support both the E&M and procedure codes. Documentation and coding functions must be completed by the end of each patient care encounter. Patient Safety Compliance and Reporting: Contract Hospitalist shall follow all established patient safety and infection control standards of care. Contract Hospitalist shall make every effort to prevent medication errors, falls, and patient injury caused by acts of commission or omission in the delivery of care. All events related to patient injury, medication errors, and other breeches of patient safety shall be reported to the COR. As soon as practicable (but within 24 hours) Contractors shall notify COR of incident and submit to the COR the Patient Safety Report, following up with COR as required or requested.Customer Service: Contract Hospitalist shall refer all patient/customer service issues to the, MAA, COR, or designee.ADMINISTRATIVE LEAD HOSPIALIST/CONTRACTOR MEDICAL DIRECTOR: The Lead Hospitalist will provide additional administrative duties outside of direct patient care functions. This list is not all inclusive and additional duties and responsibilities may be added, as required by VA.Meetings: When requested, the Lead Hospitalist shall attend staff meetings as required. Lead Hospitalist to communicate with COR and report any conflicts that may interfere with compliance of this requirement. Lead Hospitalist is required to attend all Quality Assurance meetings.Oversee the routine operations of contract staff to include administrative and professional oversight;
