Age Well Being Coordinator Roehampton and Tooting (Garratt Lane) Pay rate : £12.37 p/h Hours of work: 31.2 hours per week (Monday to Friday, mostly office hours, but some flexibility required to work occasional evening (s) Temporary - we require the successful candidate to commit for a period of 3 month (minimum), with possible extension Main purpose of the post: work with the community offering access to the range of health and wellbeing activities in the Roehampton area of Wandsworth to support older people to maintain their independence Key Responsibilities: Ø Actively engage and work in partnership with statutory, voluntary, and independent sector Ø services, and residents aged 55 plus themselves, to optimise local resources and local knowledge in developing service provision. Ø Support older people from Roehampton to use health and wellbeing services and link them into their wider communities Ø Reach older people who may be at risk of losing their independence and offer support to either aid recovery or delay deterioration in their health & wellbeing. Ø Continue to develop Age Well services by engaging in provider forums and sharing best practice and expertise. Ø Adopt an asset-based community development approach that maximises service users personal resources, strengths and informal networks including families and friends to achieve their goals and promote resilience. Ø Continue facilitate relationships and effective referral mechanisms for relevant professionals and stakeholders including statutory health and social care services Ø Coproduce programmes of activity to support the fostering of community agency by enabling community members to come together to create change and realise a sense of control in individuals to help improve their health. Ø Support older people and other residents to initiate and run their own groups close to them homes based around the interests of their members. Ø Recruit and support / supervise volunteers, making best use of volunteers across the programme Ø Outreach into (some) people s homes and, where appropriate into hospitals for regular service users who are in hospital for extended periods of time Ø Explore ways to embed digital solutions into Health and Wellbeing Services as appropriate Ø Deliver peer support opportunities in a variety of ways including one-to-one advice, on-line support, or face-to-face forums Key Skills v Research Skills and proven ability in partnership working v Excellent communication skills, both written, verbal, and digital. The successful candidate will be expected to contribute to writing of monitoring reports, write case studies, and will be responsible for maintaining an Excel database with key data for monitoring reports, as well as keep service user records up to date (using a database) v The successful candidate may also be asked to attend contract monitoring meetings with the Service Manager and / or area manager v Self-starter - ability to work under one s own initiative and find creative solutions to support isolated community groups v Knowledge and experience to set up relevant and attractive services for older population v Understanding of the health needs of older people including issues of dementia, falls prevention, social isolation v Ability to develop and maintain professional working relationships with a wide variety of stakeholders including community services, customers, professionals, from statutory and voluntary organisations and members of the public v Knowledge and understanding of safeguarding issues and ability to address them appropriately v Experience of recruiting, supervising, motivation and supporting volunteers v Experience of co-ordinating Age Well /Public Health programmes - desirable v Knowledge of community development work desirable
